245D Waivered Services

Accra is a 245D licensed provider of respite, homemaker, Individual Community Living Support (ICLS) services, Individual Home Supports (IHS) without training, IHS with training, and night supervision services. For each 245D service, clients select a trusted, qualified worker suited for their needs, while for IHS with training, a qualified staff is matched to the client to meet their needs. In all services, an assigned 245D Accra Service Coordinator will work with the client and county case manager to set up and administer 245D Waivered Services.

Accra also administers Caregiver Living Expenses, which provide a reimbursement of certain expenses, such as rent and food, incurred by an unrelated caregiver who is living in the same household as an individual receiving waivered services and providing approved services.

Individualized Home Supports (IHS) With Training

Individualized Home Supports (IHS) with Training services provides skill-building and instructional services, including locating and accessing community resources, to an adult looking to acquire, retain, and/or improve the skills needed to enhance their experience living in the community.  Individuals receive Training and Support from an assigned Accra staff, who is qualified to meet their needs.  Individuals accessing IHS with Training receive support and training in at least one of the following Community Living Service Categories:

  • Community Participation
  • Health, Safety, and Wellness
  • Household Management
  • Adaptive Skills

How does Accra select the IHS Training staff who will assist me?

Accra considers several factors when selecting your staff, including location, availability, and your own personal preferences. 

What Types of activities could my IHS staff assist me with?

Your IHS with Training staff can assist with a variety of activities.  Accra will work with you, and your assigned Waiver Case Manager, to develop a service delivery plan that is tailored to meet your accessed needs and unique interests.

How does IHS with Training differ from IHS without training?

While IHS without training is a Basic 245D Service primarily focused on health and safety, IHS with Training is an Intensive 245D Service with also focuses on skill building, promoting independence, and connecting individuals with community resources.

Individualized Home Supports Without Training

Individualized Home Supports Without Training services provide support, assistance and supervision to adults or children who live in their own homes. A staff person provides direct supervision, cueing, guidance, instruction, assistance with activities of daily living, or assistance with coordinating and attending community living activities. Individualized Home Supports Without Training services can be provided in the person’s own home or community.

Individualized Home Supports Without Training was created through the State of Minnesota Waiver Reimagine project, and it combines the previous services of Personal Support and Adult Companion.

I have heard of PCA services. How is Individualized Home Supports Without Training different?

While an Individualized Home Supports staff can provide all the same care as a PCA, they are also focused on helping the client to increase their independence and access their community.

Can a parent work as an Individualized Home Supports Without Training staff?

Yes. Parents can work as an Individualized Home Supports Without Training staff, as long as the client is an adult.

Can a parent be both an Individualized Home Supports Without Training staff and the Responsible Party for the client?

Yes. Under Individualized Home Supports Without Training, the person acting as the client’s Responsible Party may also work as a staff.

Respite

Respite FAQ

Respite is a short-term care service intended to provide a break to an individual’s primary caregiver. Respite may be provided in the individual’s home or the respite provider’s home.  This service is available under various payment options including, Medical Assistance Home and Community Based Services (waiver programs), and private pay.

How does Respite work? How long can my Respite breaks be?

Respite is intended as a break for the client’s Primary Caregiver(s). Depending on the number of Respite hours available, this break could be, for example, for a few hours, or even an entire weekend, including overnights.

Does this mean my Respite staff could work overtime?

While Respite staff are not allowed to work overtime on a consistent weekly basis, Accra does recognize that, based on the intention of the service (a break for the Primary Caregiver), there may be times when a respite staff member is needed to cover a longer than typical break. Your assigned Accra 245D Service Coordinator will help you understand when overtime is an option under Respite Services.

Homemaker Services

HOMEMAKER SERVICES FAQ

Homemaker Services range from light household cleaning and laundry to assistance with home management and activities of daily living. Home Management includes:

  • Meal prep
  • Shopping for food, clothes and supplies
  • Simple household repairs
  • Arranging for transportation

Homemaker Services are available under various payment options, including Medical Assistance Home and Community Based Services (waiver programs) and private pay.

What is the difference between Basic Homemaker Services and 245D licensed Homemaker?

Basic Homemaker Services are not a 245D licensed service, and they allow for a staff person to assist with cleaning and laundry.

There are two types of 245D licensed Homemaker Services:

  • Homemaker/Home Management Services allow for a staff person to assist with cleaning; laundry; meal prep; shopping for food, clothes and supplies; simple household repair; and arranging for transportation.
  • Homemaker/Assistance with ADLs allows for a staff person to assist with cleaning, laundry, and assistance with some physical care.
Is there a different hiring process for Basic Homemaker and 245D licensed Homemaker Services?

Yes. While all Accra Homemaker employees must complete an Accra employee application and pass a state required background check, only Homemakers working under the 245D license are required to complete an additional 245D Employee Training Packet.

How does a person qualify for Homemaker Services on a Waiver?

There are two scenarios where a person may qualify for Waivered Homemaker Services:

  • The person is unable to manage the general cleaning and household activities of their own home.
  • The Primary Caregiver(s) who is regularly responsible for these activities is unable to manage them or is temporarily absent.

Night Supervision Services

NIGHT SUPERVISION FAQ

Night Supervision is a non-preventative service consisting of overnight assistance and monitoring provided by a staff in the client’s home when the client has an assessed need in one of the following covered service areas:

  • Assistance with activities of daily living (ADLs)
  • Assistance with instrumental activities of daily living (IADLs)
  • Implementation of positive support programming and transition plans
  • Reinforcement of skill development supports
Does a Night Supervision Staff have to be awake if the client is sleeping?

Night Supervision staff do not need to be awake if the client is sleeping. However, caregivers who live in the same home as the client may not be paid for any time the client is asleep.

How much does a Night Supervision Staff make?

Night Supervision starting wages vary between $18.75 – $19.75 depending on which region of Minnesota the client lives. The Night Supervision starting wage in the metro region is $19.75.

Individual Community Living Supports (ICLS)

Individual Community Living Supports (ICLS) offers assistance and support for older adults who need reminders, cues, intermittent/moderate supervision, or physical assistance to help them remain in their own homes. Individual Community Living Supports covers assistance and support for eligible people 65 years and older in the following service categories:

  • Active cognitive support
  • Adaptive support service
  • Activities of daily living (ADLs)
  • Household management
  • Health, safety and wellness
  • Community engagement

Individual Community Living Supports (ICLS) services are available to people on the:

Who determines which of the six ICLS Service Categories the individual qualifies for assistance in?

Your county case manager will supply Accra with a State of Minnesota Individual Community Living Support (ICLS) Planning Form. The planning form indicates which ICLS Service Categories the individual requires assistance in. Accra uses this planning form to develop, with the individual, their Coordinated Services and Support Plan (CSSP) Addendum.

Who can provide Individual Community Living Support (ICLS) services?

Relatives and non-professional legal guardians/conservators are allowed to provide ICLS services. However, the ICLS Direct Support Staff may not be the person’s spouse, professional legal guardian/conservator, landlord, or have any financial interest in the person’s housing.

Are Accra ICLS Direct Support Staff able to perform all the supports listed in the six ICLS Service Categories?

Accra ICLS Direct Support Staff are able to perform all supports listed in the six ICLS Service Categories, with the exception of medication set-up and administration. Accra ICLS Direct Support Staff are trained in medication assistance.

CAREGIVER LIVING EXPENSE FAQ

Caregiver Living Expenses

The Caregiver Living Expenses program provides a reimbursement of certain expenses – specifically rent and food – incurred by a caregiver who is living in the same household as an individual receiving waivered services and providing approved services. Caregiver Living Expenses are available to people on either the Developmental Disabilities (DD) Waiver or the Community Access for Disability Inclusion (CADI) Waiver.

Who determines if my roommate is eligible for a Caregiver Living Expense reimbursement, as well as the amount of the monthly reimbursement?

Your County Case Manager will supply Accra with a State of Minnesota Caregiver Living Expenses Worksheet, which informs Accra of the name of the authorized live-in caregiver, as well as the amount of the monthly reimbursement.

Are there any restrictions on who can be my authorized live-in caregiver?

Caregiver Living Expense reimbursements are only available to individuals who are NOT related by blood, marriage, adoption or are otherwise legally responsible for the client.

Is this payment an “advance” on rent?

Caregiver Living Expense is a reimbursement of certain expenses, including rent. It is not an advance on someone’s rent. The authorized live-in caregiver does not become eligible for each month’s reimbursement until they have worked at least one shift of their authorized service within the month they are seeking reimbursement for.
